Students must seek prior approval for any activity as it needs to be reviewed and approved by both Student & Clinical Affairs.

UMSOD students and student led organizations and clubs interested in participating in any type of event or activity, must complete the UMSOD Event Registration Form.  The Student Activities Form should be completed and uploaded into the request form. Please allow at least two weeks for your event to be reviewed and approved by the Office of Student Affairs.
