Facilities Request
Facilities requests can be submitted by using the “GET HELP” tab on your desktop; click on the request category tab and select facilities. You can also send an email to dl-dental-facilities@umaryland.edu.
Key Requests
The requesting department must include a SOAPF when requesting a key. The Lock Shop will not accept any requests for new keys unless a SOAPF is provided to charge for the key. Office keys are $18.23 per key and cabinet keys are $15.37 per key.
Keys must be requested by a supervisor.
Gender Neutral Restroom
A gender neutral restroom is located on the 4th floor, Room 4318.
Parking
To apply for parking or to update vehicle information:
Complete the online DocuSign parking application. Make sure to add Joe Sabbat in the parking liaison section of the parking application in order for it to be received and processed timely.

Parking cancellation
A parking cancellation form must be submitted in order to cancel a parking space. Parking permits must also be returned to Parking Services cashier’s office to complete the cancellation process. If the permit is not returned, you will continue to pay for the parking space. If a permit is lost or unable to be returned, please contact the Parking Cashier’s office directly.

Lost OneCard
Contact the OneCard office to schedule an appointment to receive a replacement card. Bring a valid driver's license, passport, or other photo ID for verification of identity when you arrive at the office. Building and parking access doesn’t transfer to your replacement OneCard. If applicable contact the Facility Manager and Parking Services to reinstate access privileges.

UMSOD Occupant Emergency Plan
Purpose
This Occupant Emergency Plan prepares University of Maryland, Baltimore (UMB) students and employees to respond if an emergency impacts your University building.

Imminent Risk to Life Safety

When an emergency affects your University building, the action you take will either be to Evacuate, Seek Shelter, or Run-Hide-Fight.
Evacuation: Using the nearest safe exit, leave the building. Use enclosed stairwells, not elevators or open stairs (e.g., those commonly found in building lobbies). Encourage others to evacuate as you leave, but don’t delay your own evacuation. If an individual asks you to help them evacuate, please do so if you feel comfortable; otherwise report their exact location to first responders.
- Assemble:
- Those who exit from the front door will assemble directly across W Baltimore
- Those who exit from the west stairwell exit will assemble in the HSF-III
- Those who exit from the north exit doors will cross Fayette St and assemble at the UMMC parking lot.
- During extreme weather, go to the Health Science Facility III and wait in the main
Evacuation Assistance: If you need help evacuating, go to the nearest enclosed stairwell.
- If you are not in imminent danger, do not block others from using the

Patient Evacuation: Patients undergoing routine dental procedures are expected to exit the building when the fire alarm is activated. If interruption of patient treatment could potentially compromise a patient’s health or seriously compromise the expected results of treatment, the treating doctor or supervising faculty member may decide to protect the patient in place, along with any necessary treatment personnel. The medical procedure can continue to be performed if there is no imminent danger. The intent of this evacuation procedure is to apply to patients undergoing surgical procedures, under sedation, or are being treated for a medical emergency. The ultimate evacuate/protect in place decision will be made by the individual responsible for the patient. The Emergency Warden must be notified when patients will be protected in place, so that they can record this information on the Emergency Warden report. If in imminent danger, call 911. Ask others to help you exit the building.
Seek Shelter: Limits your exposure to emergencies outside your building.
- Go inside if you are
- Find a safe location for the type of emergency (see Hazard-Specific Protective Actions).
Run, Hide, Fight: Act quickly if it appears there is an active assailant.
- Run away to avoid the attacker if it is safe to do
- Hide if escape is not Lock and barricade doors. Avoid windows. Silence phones. Turn off lights.

Automated External Defibrillators (AEDs) ▾
AEDs are found across UMB and can be used by anyone. If someone is experiencing a cardiac emergency:
- Yell for Tell someone nearby to call 911. Ask that person or another bystander to bring you an AED if there is one nearby. Time is of the essence.
- When a person is passed out and not waking up, check their If the person is not breathing or is gasping, begin CPR.
- Perform CPR by pushing down hard and fast in the center of the chest at least 2 inches at a rate of 100 to 120 pushes per minute, allowing the chest to come back up to its normal position after each push.
- Use the AED as soon as it Turn it on and follow the prompts.
- Continue administering CPR until the person starts to breathe or move, or until someone with more advanced training takes over, such as first responders.

During an active assailant incident, try to run away from the attacker if it is safe to do so. If escape is not possible, hide. When hiding, secure your location by locking the door. Below are the different types of locks you may find on campus:
