Enrollment Verification

If you require enrollment verification please contact the Office of the Registrar or the Office of Academic Affairs for processing. The Office of Academic Affairs does not perform verifications over the phone. Please be sure to attach a release form when submitting the verification request. Forms may be faxed to 410-706-0406.

Application for Licensure or Degree Certification

Applications for Licensure that require the Dean's signature and School of Dentistry seal should be sent to the Office of Academic Affairs, Dean's Office, 6th Floor. #6421, 650 W. Baltimore St. Baltimore, MD 21201.

Requests for Degree Verifications should be sent to the Office of the Registrar.

Dean's Evaluation Letter

Evaluation Letters are provided to students or alumni applying to postgraduate programs. Letters are processed by the Office of Student Affairs.  For more information, please contact Ms. Debbie Suls or Ms. Memory Jackson.

Replacement Certificate

If you require a replacement postgraduate certificate, please contact Ms. Theresa Pate or the Office of Academic Affairs to request the necessary form. If you require a duplicate diploma or transcript, please contact the Office of the Registrar.

Absence Notification

Students who miss educational activities must complete an absence form. The form may be obtained from the course director or the Office of Student Affairs.
