Information Technology Procedure: axiUm swipe card/password management PURPOSE The purpose of this document is to inform University of Maryland Dental School Information Technology personnel of the procedures for magnetic encoding card management for the clinical and academic application (axiUm). Magnetic Encoding Card (swipe card) Management As a HIPAA security policy, identification cards with magnetic encoding (here referred to as swipe cards) are reset and distributed to users every fiscal year. This process requires the generation of a set of new passwords by axiUm a few weeks before the activation of the new password and swipe card. The newly generated passwords will be processed by the swipe card printing software and a new set of swipe cards will be printed with the user name and encoded with the new password. This set of new swipe cards will be distributed to their owners about two weeks before the expiration of the old cards and the activation of the new passwords. Users who do not use swipe cards but are affected by the new password change will also be prompted every 90 days in Axium. Lost Card If a swipe card is lost, stolen, or just not with you, it can be replaced by visiting the Information Learning Center (ILC) at the ground floor of the Dental School. You will need to show a valid pictured identification in order to receive a new card. Your old card will no longer work. Instructions for Changing Axium Password Axium Password is sensitive to CAPS LOCK setting. Axium password rules: Password must be 8-10 characters... Password should not use any part of your name... Password must contain ALL of the following classes of characters: * Uppercase letters (A,B,C,...) * Lowercase letters (a,b,c,...) * Special characters ($,#,@,...) EXCEPT asterick * * Arabic numerals (1,2,3,...) Note: Axium will enforce password changes every 90 days. Swipe Cards remain functional.
Password Reset If you forget your password or you believe your password has been compromised, please request a password reset in ILC. Passwords should NOT be shared with anyone, not even your supervisor.
